Your initial appointment usually will consist of an examination and review of your treatment options. Usually, treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information prior to your first visit:
- Any recent x-rays taken by a previous dentist (usually 6 months or less), if applicable (please call the office at Cupertino Office Phone Number 408-996-7001 for email instructions).
- A list of medications you are presently taking (may also be provided directly to Dr. Pan during initial consultation)
- If you have dental insurance, we ask that you provide the name of the insurance company, group number, ID number and policyholder’s and patient’s date of births in order for us to retrieve your dental eligibility benefits online or over the phone ahead of time.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Basic x-rays will typically be taken at our office for new or first time patients (full mouth x-rays or panoramic x-ray will only be taken on an as needed basis). Even with recent x-rays transferred from a previous dental office additional x-rays may still need to be taken at our office due to image or diagnostic quality issues.
Please note that our office uses complete digital x-rays which tremendously increases image quality, diagnostic precision and reduces radiation.
